Maine COVID-19 Certificate of Compliance for visitors from outside ME-NH-VT

The Mills Administration released yesterday the latest version of its "Certificate of Compliance for Maine Visitors" form which persons from outside ME-NH-VT must complete and provide to the operator of any commercial lodging establishment - including hotels, campgrounds, short-term rentals, etc. - at check-in.

Key points:

Visitors are asked to attest that:

  1. They have not experienced or displayed any of several specific COVID-19 symptoms in the past 24 hours
  2. They have not in the past 14 days had close contact with anyone who has a confirmed case of COVID-19
  3. Any one of the following statements is true:
    • They have received a negative COVID-19 test result on a specimen taken not more than 72 hours prior to arrival in Maine
    • They agree to quarantine for 14 days upon arrival or for the duration of their stay
    • They have already completed a 14-day quarantine in Maine
  4. They agree to follow recommended safety measures while in Maine
  5. All persons in their care - under age 18 or otherwise - meet the criteria of Items 1 and 2 (above). (List names and ages.)

Also:

  • Persons under age 18 - and residents of ME, NH or VT - are exempt from completing the form.
  • Visitors may be tested for COVID-19 in Maine, but must remain in quarantine until receiving a negative result.
  • Visitors may be asked to furnish proof of a negative test on request.

Persons completing the form must sign and date it, and provide an address, home phone, and contact phone while in Maine.

Note that Friday 26 June is the first day when Maine commercial lodgings can accept guests from outside ME-NH-VT.
